This is a recurring problem. I have heard of tanks becoming unscrewed from their valves. Not pretty.

This is true. It is my opinion that they do this as well as serve as a warning of what is to come. You would have to be severely stupid to continue unscrewing a tank that has come loose from the valve. I know that if I was to have one unthread so far that it started leaking, I sure as hell wouldn't continue. Not that I would let it get that far.
