Yes - Given a person with a medical condition by which normally "safe" blows may not be safe, and given a proper speed and a proper (or not so for the target) angle, precise hit (or a volley of hits to that region).
I forget the term for the condition of which I speak, but go watch the Rocky movies and you'll know what I'm talking about.
No - Given any realm of normalcy, a paintball is not going to have the force or mass needed for a lethal hit. The chances of the above stated scenario are very remote.

Funny thing though is that your basic mask and goggles protect those areas. A lot of people don't wear a neck protector but I can imagine a hot shot to the voice box could cause you to suffocate.
The thing is that if you play within the accepted manner you will have NO problems. Safety rules are there for a reason.

Other locations, no, I doubt it. Maybe if one went right down your throat, it could cause excessive bleeding or choke you. But a shot to the eye is potentially fatal, which is why we have to be so hardcore about wearing masks (As in, if you see someone without a mask on on a field, tackle them to the damn ground.)
I've taken hits to the throat at close range with appropriately-chroned markers. It hurt like hell, but caused no damage to my windpipe.
